其實驗設計如下, 一樣也是phase III RCT, 共收案518個病人, 分成兩組, HA-WBRT+memantine v.s. WBRT + memantine, 結果發現HA-WBRT+memantine明顯認知功能下降得比較慢
Memantine給法也值得學起來, 有兩種給法:
1. week 1: 5 mg at morning, week 2: 1.5 mg BID, week 3: 10 mg at morning, 5 mg at evening, week 4-24: 10 mg BID
2. week 1: 7 mg QD, week 2: 14 mg QD, week 3: 21 mg QD, week 4-24: 28mg QD
當然, 身為一個放射腫瘤科醫師, 除了知道實驗結果外, 也很關心放射治療技術, 基本上跟RTOG 0933 有一點點差別, 劑量同樣是給30Gy/10Fr,
PTV= whole bran paremchyma - hippocampus avoidance region,
Dose constraint:
PTV: D2% <= 37.5Gy, D98%>=25Gy, V30Gy>=95%(RTOG0933是>90%)
Hippocampus: D100%<=9Gy, Dmax (0.03 cc volume)<=16Gy (RTOG 0933也是限制這個劑量, 但是沒看到最小計算體積是多少)
Optic nerve and optic chiasm: Dmax(0.03 cc volume) <= 30Gy (RTOG 0933是37.5Gy,而且沒看到最小計算的體積是多少....)
