[肺癌] 早期肺癌的立體定位放射治療

針對早期肺癌的立體定位放射治療, 目前許多臨床試驗, 例如RTOG 0618 跟 RTOG 0813 有發表結果, 其中 RTOG 0618 是針對周邊的肺癌做立體定位放射治療, 而 RTOG 0813 是針對位於中間的肺癌進行立體定位放射治療, 以下就來細讀吧!

1. RTOG 0618: 

(1) single arm, phase II trial, peripheral T1-T2N0M0, 符合可開刀條件, 切片已證實,FEV1 and diffusing capacity > 35% predicted,  arterial oxygen tension > 60 mm Hg, arterial carbon dioxide tension < 50 mm Hg

(2) RT:  54Gy/3Fr in 1.5-2 weeks

(3) 結果:  發表過兩次

a. ASCO abstract (2013): 共收案33個病人, 最後只有26個病人可以評估, 2-year estimates of PFS: 65.4% (95% CI: 44.0%, 80.3%); 2-year estimates of OS: 84.4% (95% CI: 63.7%, 93.9%)

b. JAMA oncology (2018):  The 4-year estimates of DFS: 57% (95% CI, 36%-74%); 4-year estimates of OS: 56% (95% CI, 35%-73%)
