[肺癌] 非小細胞肺癌的術後放射線治療的角色

非小細胞肺癌一般來說, 如果術前評估分期是IIIA以前, 會考慮進行手術,  手術後一般來說會依照病理情況, 決定後續治療, 一般來說margin positive 或是pN2 才會考慮進行術後放療, 所以臨床上遇到肺癌病人來做術後放療的少之又少.....

針對肺癌術後放療的研究相當的多, 考試也喜歡考, 因此這邊筆者就來整理筆記方便之後再做閱讀

一. Lancet(1998) 的統合分析: 這篇統合分析非常有名, 基本上把肺癌術後放射治療的角色否定掉! 其作法非常的厲害, 使用九個RCT的individual patient data (這個有在做統合分析的人就會知道, 要能夠要到資料非常的困難, 一次能夠要到九個基本要大師才能做到....), 總共收集2128個病人, 裡面有1056個病人接受術後放射治療, median follow-up 為3.9年, 結果發現術後放射治療對overall survival 有明顯壞處, hazard ratio:1·21 [95% Cl 1·08–1·34]; subgroup analysis 發現 N0-1, stage I/II的病人明顯影響到overall survival, N2, stage III的病人沒有定論! => 不過這篇統合分析被批評有太多病人是pN0(25%), 許多病人是用Co-60治療, 許多舊的研究分期survey不夠完整

其團隊於2005年發表update的統合分析, 其結果類似(連結點此) 

二. 近代使用 LINAC 的相關研究:

1.  JCO 2006: 為SEER database 的研究, 發現PORT 在 pN2對病人的overall survival 有幫助, 但是在 pN0, pN1沒有

2.  ANITA subgroup analysis: 這個是一個RCT的次分析(subgroup analysis), 本來的研究設計是希望在 stage IB - III 的病人, 接受手術後, 完全切除乾淨(R0 resection),分成兩組, 一組接受化療(cisplatin and vinorelbine), 另一組觀察, 如果病人pN+的話, 可以考慮讓病人接受放療(沒有強制), 因此可以做次分析, 結果發現術後放療對pN2病人的OS有幫助, 對pN1的病人有壞處!

3. JTO 2015: NCDB 研究 (2004-2006), 針對 pN2病人, 有接受化療, 結果發現PORT對OS有幫助

4. JCO 2015: NCDB 研究 (2006-2010), 針對完全切除, pN2病人, 有接受化療, 結果發現PORT對OS有幫助

5. JTO2017: 韓國的phase II study, 針對stage IIIA, pN2, 接受手術後完全切除的病人, 術後接受CCRT(n=51) versus CT (n=50), 發現 DFS 和 OS 在兩組沒有差別

6. Lung ART trial: 直接挑戰guideline, 針對 pN2, stage II or IIIA 的NSCLC, 接收手術後,完全切除乾淨的病人, 一組接受術後放療, 一組沒有. 其初步結果發表在 ESMO 2020. 共收案501人, 252人接受PORT, 249人觀察, 結果發現不管在DFS 或者是 OS 都沒有幫助

2022 更新的結果發表在Lancet, 發表結果也是PORT對disease free survival 沒有幫助, 副作用的部分: The most common grade 3-4 adverse events were pneumonitis (13 [5%] of 241 patients in the PORT group vs one [<1%] of 246 in the control group), lymphopenia (nine [4%] vs 0), and fatigue (six [3%] vs one [<1%]). Late-grade 3-4 cardiopulmonary toxicity was reported in 26 patients (11%) in the PORT group versus 12 (5%) in the control group. 

7. PORT-C trial(JAMA oncology 2021): 中國的研究,  364個病人, pN2, IIIA, NSCLC, 手術完全切除乾淨的病人, 分成兩組, 一組接受術後放療, 一組觀察, primary endpoint 是 DFS, 結果發現3-yr OS, 3-yr DFS 都沒有差異, 3-yr LRFS在PORT組比較好, 但是3-yr LRFS並不是主要的endpoint, 所以要不要做PORT可以說是還沒有定論!
