[肝癌]肝細胞癌預後 - BCLC stage

最近遇到不少肝細胞癌的病人, 其中肝細胞癌預後有一個很重要的因子, 被稱作BCLC(Barcelona clinical staging), 以下就來簡單介紹一下:

可以分為 5 個 stage:

Stage	肝腫瘤	肝功能	ECOG
0    very early stage	1顆, <=2cm	Child-pugh A	0
1        early stage	1顆, > 2cm   2-3顆,<= 3cm	Child-pugh A-B	0
2  intermediate stage	>3顆 or >=2顆(any>3cm)	Child-pugh A-B	0
3    advanced stage	侵犯portal vein Or 肝外轉移	Child-pugh A-B	1-2
4     terminal stage		Child-pugh C	3-4

其中 Child-Pugh score 的評分標準為:

Score	Bilirubin(mg/dL)	Albumin(g/dL)	PT(s)	Hepatic encephalopathy	ascites
1	<2	>3.5	<4	none	none
2	2-3	2.8-3.5	4-6	1-2	mild
3	>3	<2.8	>6	3-4	severe

把五項的分數加總, class A(5-6分), class B(7-9分), class C(>9分)

而hepatic encephalopathy 的評分標準為:

grade 1:  attention deficit, tremor
grade 2:  asterixis(把手掌彎曲,會顫抖),confusion,lethargy,ataxia
grade 3:  rigidity, clonus, hyporeflexia, somnolence
grade 4:  coma

最後就是 ECOG的評分惹:

grade 0: 生活機能跟發病前完全一樣
grade 1: 生活機能稍微受限
grade 2: 可以自我照顧,但無法做任何工作
grade 3: 只能部分自我照顧,躺在床上時間>50%清醒時間
grade 4: 完全臥床
grade 5: 死亡
