[乳癌] 停經的定義

乳癌在臨床試驗裡面, 有一個很重要的定義要做釐清, 就是停經的定義, 因為停經後, 女性的雌激素會明顯下降, 而且乳癌荷爾蒙療法的aromatase inhibitor 也是要在停經後用才會比較有效果(詳細原因牽涉到生理機轉, 有機會再寫....)

在NCCN guideline裡面對停經下了定義, 以下有幾個情形可以算是停經:

1. 之前接受過雙側卵巢輸卵管切除(bilateral oophorectomy)
2. 大於等於60歲
3. 小於60歲且停經大於等於十二個月, 並且沒有使用化療, tamoxifen, toremifene or ovarian suppression; 且FSH 和estradiol要在停經後的範圍
4. 如果有吃tamoxifen, toremifene, 而且小於60歲, 則FSH 和estradiol要在停經後的範圍

在這邊NCCN guideline特別強調如果病患有在使用LHRH angonist or antagonist,  就不能說病患停經; 另外如果是因為化療導致的無月經, 那不能說病患停經,  因為卵巢功能可能還是完好的
